ELC COKATO'S RESERVATION PROCESS
Please read the following terms and conditions for Non-Profit and Profit Usage. A.) Application Request (Non-Profit). B.) Application Request (For Profit/Other Events).
A.) APPLICATION REQUEST (NON-PROFIT)
1.) ELC Cokato members may request to use the church facilities free of charge for non-profit activities. The member who makes the reservation is expected to be at the church during the event and be the responsible party.
2.) Non-members of ELC Cokato may request to use the church for a fee of $50.00 for three (3) hours of use. Additional time may be requested at $16.00 x hour.
3.) Each usage request requires that a building use room request form be completed.
4.) All groups requesting to use the church facilities for events/activities not affiliated with the ELC Cokato will be required to sign the liability disclaimer form located on the following page.
5.) Please indicate on the form if this is a recurring event as a one-time fee may be negotiated (for one calendar year) if the church’s expectations are followed and met each time.
6.) The church office is responsible for reviewing all forms and for updating the master calendar.
B.) APPLICATION REQUEST (FOR PROFIT/OTHER EVENTS)
1.) Each usage request requires that a building use room request form be completed.
2.) All groups requesting to use the church facilities for events/activities not affiliated with the ELC Cokato will be required to sign the liability disclaimer form located on the following page.
3.) Please indicate on the form if this is a recurring event as groups requesting recurring time can negotiate a flat rate on a per month or per year basis.
4.) The church office is responsible for reviewing all forms and for updating the master calendar.
5.) It is mandatory to pay the on-site custodian charge when you are “renting” any part of the church for-profit events - On-Site Custodian... $48.00 for 3 hours and $16.00 for each additional hour.
6.) The following fee schedule applies to people who are requesting the church facilities for events where a profit may be made or business is performed. The rate designated will grant you 3 hours of use. You may request additional time as noted.

MANDATORY CHARGE
C.) APPLICANTS NEED TO KNOW
1.) You must provide adequate supervision and the person submitting the application will be held responsible for the group and its actions.
2.) If your event takes place outside of ELC Cokato normal business hours (Monday-Thursday 9:00 AM – 3:00 PM), the office will assist you with unlocking the building when you arrive and locking the building when your event is over.
3.) All events must be concluded by 9:00 PM unless prior approval has been given.
4.) Please label ALL items you bring into the church. This can be very helpful for returning lost items.
5.) Cake pans, serving dishes, etc. left in the kitchen will be put on the coat rack shelf in the downstairs hall by the south entry. After one month, any item not claimed will be donated or properly disposed of.

D.) EXPECTATIONS OF MEMBERS & NON-MEMBERS
For using the facilities of ELC Cokato.
1.) The event must not conflict with our Christian Mission.
2.) Smoking and Alcohol are not permitted within ELC Cokato or on the grounds.
3.) Return the facility and its equipment to its original condition.
4.) Extra equipment brought in for serving should be taken out after the event.
5.) Tables and countertops need to be wiped clean with Lysol disinfecting spray. If table decorations were removed prior to your event, please put them back on the table before you leave the building.
6.) All dishtowels used are to be laundered AND RETURNED THE NEXT BUSINESS DAY.
7.) Please put all garbage in appropriate waste baskets. The church custodian will take care of waste baskets that are full. Please recycle as much as possible.
8.) Carefully package any needles/sharps that were used during your reservation and bring them with you when you leave. Please DO NOT place any needles/sharps in the garbage.
9.) Please turn off the lights before you leave.

AGREEMENT OF RELEASE AND WAIVER OF LIABILITY
By signing this form the signee is declaring that he/she is the “renter” of ELC Cokato.
“Renter” warrants and represents that he/she is able and hereby assumes full responsibility for the care and safekeeping of all property brought into the premises.
“Renter” warrants and represents that he/she is able and hereby assumes full responsibility for any injury to persons or damage to property occurring as a result of the “renter’s” use of the ELC Cokato.
“Renter” agrees to indemnify and hold harmless ELC Cokato against any claims arising out of or relating to this agreement.
“Renter” agrees to indemnify and hold harmless ELC Cokato against any claims arising out of or relating to this agreement.
“Renter” agrees to take good care of the ELC Cokato, and upon completion of rental shall restore the church to its condition prior to the “renter’s” entry.
