Easter Garden Plant Order Form

If you wish to purchase a plant, please fill out this form and return it to the church office by 9 AM on Thursday, April 10th, you may pay in the app under "Give" then select "Altar Flowers".

Please make checks payable to Evangelical Lutheran Church. You can drop-off your payment and order form during regular office hours or mail to PO Box 448, Cokato MN 55321.

You may share your flowers from anywhere. If you are bringing your plant NOT purchased through the church office, notify us at 320-286-5964 no later than 9 AM on Thursday, April 10, 2025, so we may print your name in the Easter Sunday bulletin.

Be sure to TAG YOUR PLANT with your name if you plan to pick it up after Easter. You may pick up your plant after the 10 AM Easter Worship Service but no later than noon on Wednesday, April 23rd.

Please indicate your first choice and an alternate choice of plants and the quantity you wish to order. If the florist is unable to provide your first choice, they will provide your alternate choice.

If you would like to honor or memorialize those special in your life, please fill out the designation below and your name and those people’s names will be printed in a special insert in the Easter Sunday bulletins.
